Please book an appointment with Susanna if you need help with form completion. This can save time both for you and your doctor. Once your forms are completed you should present the form to the Front Desk to determine if you need an appointment with your doctor or if your doctor can complete it without an office visit.

PLEASE NOTE: Forms, letters for insurance, as well as sick notes are not covered by the Medical Service Plan. Your physician charges a reasonable fee for form completion. For more information, please go to Frequently Asked Questions.

Due to confidentiality reasons we are unable to email any completed forms, letters, lab requisitions or prescriptions.

SICK NOTES/RETURN TO WORK NOTES:

There is a charge for sick notes/return to work forms as this service is NOT covered by MSP.
