We are once again able to provide letters and complete forms. Forms, letters for insurance, as well as sick notes are not covered by the Medical Service Plan. Your physician charges a reasonable fee for completion of same. For more information, please go to Frequently Asked Questions.
Due to confidentiality reasons we are unable to email any completed forms, letters, lab requisitions or prescriptions.

SICK NOTES/RETURN TO WORK OR SCHOOL COVID19:
Please note as per Dr. Bonnie Henry and Mr. Adrian Dix, employers have been informed that they are not to require sick notes for absences due to illness, nor for return to work after illness. As per guidelines from BCCDC and the Medical Health Office a note is NOT required if an individual develops new symptoms of possible Covid19. A note is NOT required for an individual to stay away from school/participate in non in-person education activities. A note is NOT required to return to school once symptoms have been resolved for more than 48 hours, and NO contact with a person with a confirmed COVID19 positive test. A note is NOT required for individuals to attend school when they are experiencing normal symptoms related to an existing condition such as allergies or chronic sinusitis or asthma. A note is NOT required to stay away from school/participate in non in-person education activities for someone with chronic illness with respiratory symptoms such as allergies, asthma, chronic sinusitis. A note is NOT required for an individual who has COVID19 symptoms but a negative test for this individual to stay home until their symptoms have been resolved for more than 48 hours. A note is NOT required for an individual who has symptoms consistent with COVID19, or had a positive COVID19 test or exposure to a person with a positive test, who has self isolated for 14 days and is now better and ready to return to school.
The ONLY instance where your Doctor must provide a note is if someone in your household is immunocompromised. In this instance, your Doctor must complete the Ministry of Education form for an IEP (Individualized Educational Plan). There is a charge for completion of this form as this service is NOT covered by MSP.
