Disability Tax Credit Forms

At this time of year, many accountants advise patients to ask their physician to complete a disability tax credit form. However, the criteria are stringent and most patients do not qualify. You as a patient must pay for completion of this form. It is now 16 pages in length, and at $20/page it could cost as much as $320 which will be charged whether you qualify or not. To check to see if you are eligible you can: complete webform at pathwaysbc.ca or book an appointment with Susanna, our social worker who will not complete the form, but can help you determine your eligibility.

New Patients

We do have three physicians at our clinic who are accepting new patients at this time, however ALL new patients are taken from the Comox Valley Health Registry. Please visit HealthlinkBC.ca/health-connect-registry and select Comox Valley to register. All physicians in the Comox Valley who are accepting patients have been advised to accept patients only from this list. This is the best chance to secure a family physician. The physicians do not accept patients who phone the clinic directly.

Did you know you can book appointments directly with our counsellor, social worker, pharmacist or Indigenous Wellness Advocate?

At Fifth Street we have a pharmacist, counsellor, social worker on staff with whom you can book appointments directly through our front desk. The pharmacist is able to help you sort through your medications and make medication suggestions to you and your doctor. The counsellor is able to see adults 18 years old and older for in person or virtual appointments for problems such as stress, anxiety, depression, bereavement and substance use. The social worker is also able to help with short term counselling, form completion, and advocate for accessing services. The Indigenous Wellness Advocate supports First Nation, Inuit and Metis patients on their journey in health and wellness.

The AFTER HOURS URGENT CARE CLINIC with whom we are affiliated offers same day, in-person, after-hours appointments Monday -Friday 5-9 pm; Saturday, Sunday and Stats from 9am-2pm

Clinic Mask Policy:

In accordance with Island Health Guidelines, masks are now only required for symptomatic individuals with any of the following: runny nose, sore throat, fever or cough. If however you feel more comfortable wearing a mask, you are welcome to do so. If you wish your physician to wear a mask, please ask!
